Facial Treatment Consent Form

I voluntarily consent to receive facial and esthetic services from Michelle's Skin Studio.

I understand and acknowledge that:

• Esthetic services are cosmetic in nature and are not medical diagnosis or medical treatment.
• I have disclosed any relevant allergies, sensitivities, medical conditions, pregnancy or breastfeeding, medications, prescription skincare products, active skin conditions, or recent cosmetic procedures that may affect my treatment.
• My treatment may include cleansing, exfoliation, extractions, facial massage, masks, dermaplaning, lymphatic techniques, LED light therapy, and other appropriate esthetic services discussed during my consultation.
• Temporary reactions such as redness, sensitivity, dryness, irritation, tenderness, breakouts, or peeling may occur.
• I will inform my esthetician immediately if I experience unusual discomfort or a reaction during treatment.
• Michelle's Skin Studio may modify, postpone, discontinue, or decline a treatment if a safety concern or contraindication is identified.
• Results vary from person to person and no specific outcome is guaranteed.
• I agree to follow the aftercare recommendations provided to me.

I confirm that I have had the opportunity to discuss my treatment and ask questions. The information I have provided is accurate to the best of my knowledge, and I voluntarily consent to treatment.
